EVERY STUDENT-ATHLETE MUST SUBMIT A NEW TISD PHYSICAL AND ALL 4 RANK ONE DIGITAL FORMS FOR THE 2025-26 SCHOOL YEAR.
Through our partnership with Houston Methodist Tomball ISD will be offering physicals on two dates.
Tomball West athletes: when signing up for physicals, select Tomball High School or Tomball Memorial High School.
• Tomball High School will be hosting physicals at Tomball High School on Saturday March 28th from 9:00am - 1:00pm
• Tomball Memorial High School will be hosting physicals on Saturday April 11th from 9:00am - 1:00pm
Physicals are $21 - Appointments are required and must be purchased online (instructions below). CASH WILL NOT BE ACCEPTED

“Download and Print” the 2026-2027 Tomball ISD Physical Form (coming soon) No other form will be accepted. This form must be filled out and completed by yourself and the physician performing the physical.
High school students should turn in their completed Tomball ISD Physical Form to the Tomball West campus athletic trainers. Junior high students should turn their completed form into their respective team coach.
